Improve Effectiveness

Historically, case management has struggled to provide consistency across multiple managers, providers, and jurisdictions. The ability for agencies to measure success in case management appears challenging at best, near to impossible at worse. And yet, best practices in behavior change point towards a core set of approaches. A map exists to improve outcomes with various assessment tools that measure how well agencies are aligned.

In the Correctional Program Checklist (CPC), the resulting score provides a measure of program integrity and quality. And yet, there are many items with which agencies struggle. Over 50% of the CPC items could be positively impacted simply through strong implementation of CorrectTech Integrated Reentry system. With expert guidance and training, CoRe Services guides agencies through key components of best practices. For example, matching individuals to case managers based on specific responsivity factors is an important practice rarely accomplished. Instead, caseloads are assigned randomly or based on caseload slot availability. CoRe Services provides the support needed to understand responsivity and leverage agency resources to meet this need.

Another key area revolves around case plan visibility and progress. Supervisors should be able to track whether cases are on track for successful reentry. However, this is rarely accomplished. Without clear tracking methods, both clients and case managers may face unnecessary difficulties. For clients, lack of progress can be frustrating and lead to disengagement. For case managers, this can lead to demoralization and burn out. However, when clear criteria are established, progress measured and outcomes reviewed, continuous quality improvements (CQI) have a chance of taking hold.

  • Following social learning theory and strength-based approaches
  • Facilitating change planning
  • Providing CBT with relapse prevention and booster sessions
  • Collaborating with community-based services
  • Monitoring referral services, observing their adherence to RNR
  • Measuring behavior change and reinforcing success
